To understand ICSI you first have to understand how normal IVF fertilises the egg. In a conventional IVF, the egg and lots of sperm are simply placed together in a dish, and the sperm does its own job of getting inside the egg, exactly like it would in the body. Nature takes its course, just outside the body.
Now in ICSI, we don't leave it to chance. The embryologist picks up one healthy sperm in a very fine glass needle, holds the egg steady with another pipette, and injects that single sperm straight inside the egg, all of this under a high-powered microscope.
So you see, the only real difference between IVF and ICSI is this one fertilisation step. Everything else - the injections, the egg retrieval, the embryo transfer is exactly the same in both. People get confused thinking ICSI is some totally different treatment. It is not. It is just one technique used within IVF.
ASRM data shows that when there is no male factor, fertilisation rates are slightly better with normal IVF. ICSI gives an advantage only when there is a real reason to use it. Using ICSI on every single IVF cycle, when there is no male factor and no other clear reason, does not improve your chances. Some large studies have even shown slightly lower implantation and live-birth rates compared to simple IVF in such cases.
