TheFertilife offers ICSI treatment in Gurgaon under the direct care of Dr. Anshika Lekhi, an infertility expert with more than 13 years of experience. Now, ICSI stands for Intracytoplasmic Sperm Injection, which is a bit of a mouthful, so most people just say ICSI. It is a laboratory step that we do as a part of IVF.

In simple words, the embryologist takes one single sperm and injects it directly inside one egg to help fertilisation happen. That's it. Most couples who need it have a male-factor problem, but it is also useful in some other situations – say, the fertilisation failed in a previous cycle, or we are planning genetic testing on the embryos.

"One thing I always tell couples: ICSI isn't automatically the right add-on for every IVF cycle. It's a powerful tool for the right indication — and an unnecessary cost for the wrong one. My job is to tell you honestly which situation you're in, not to default to the more expensive option." — Dr. Anshika Lekhi

What is ICSI and How is It Different From IVF?

To understand ICSI you first have to understand how normal IVF fertilises the egg. In a conventional IVF, the egg and lots of sperm are simply placed together in a dish, and the sperm does its own job of getting inside the egg, exactly like it would in the body. Nature takes its course, just outside the body.

Now in ICSI, we don't leave it to chance. The embryologist picks up one healthy sperm in a very fine glass needle, holds the egg steady with another pipette, and injects that single sperm straight inside the egg, all of this under a high-powered microscope.

So you see, the only real difference between IVF and ICSI is this one fertilisation step. Everything else - the injections, the egg retrieval, the embryo transfer is exactly the same in both. People get confused thinking ICSI is some totally different treatment. It is not. It is just one technique used within IVF.

ASRM data shows that when there is no male factor, fertilisation rates are slightly better with normal IVF. ICSI gives an advantage only when there is a real reason to use it. So the costlier option is not always the better one for you. What suits you will depend on your semen analysis and your earlier cycles.

Do You Actually Need ICSI?

This is a question most clinics skip. But it is an important one. It affects both your result and your cost, so please do ask it.

ICSI is clearly the right choice when there is:

  • Low sperm count, poor sperm movement, or abnormal sperm shape
  • Azoospermia, that is, where sperm has to be taken out surgically
  • A previous IVF cycle where fertilisation failed or was very low
  • Frozen or thawed sperm or eggs, where normal IVF fertilisation tends to be lower

A plan for preimplantation genetic testing (PGT), because here ICSI keeps stray sperm DNA from contaminating the genetic sample

And then there are some grey-area cases where ICSI may help, decided one by one - unexplained infertility, older mothers, or a cycle that gave us very few eggs.

Now there is one more thing you should know. Big fertility bodies like the ASRM have said it very clearly. Using ICSI on every single IVF cycle, when there is no male factor and no other clear reason, does not improve your chances. Some large studies have even shown slightly lower implantation and live-birth rates compared to simple IVF in such cases. So when a clinic puts ICSI on your bill "just to be safe", you should ask them why. At TheFertilife, Dr. Lekhi takes that call from your actual semen analysis and your history. It is not added to every cycle as a default.

What Does the ICSI Process Involve?

Most of the journey is just the IVF journey. Step by step, this is roughly how it goes.

  1. Consultation and testing: First we do a semen analysis, a hormone panel and an ultrasound, before the cycle even starts, just to confirm ICSI is genuinely the right approach for you.
  2. Ovarian stimulation: Then come the hormone injections to grow several mature eggs, tracked with ultrasound and bloodwork, usually over about 10 to 12 days.
  3. Egg retrieval: The eggs are collected under light sedation in a small outpatient procedure. You go home the same day.
  4. Sperm preparation: On the same day as the retrieval, the sperm is collected (or taken out surgically if that is needed) and the healthiest one is picked out under the microscope.
  5. ICSI injection: A few hours after retrieval, that single chosen sperm is injected into each mature egg.
  6. Fertilisation check: The very next day, the embryologist confirms how many eggs have fertilised properly.
  7. Embryo culture: The fertilised eggs are then grown in the lab for 3 to 5 days, until they reach what we call the blastocyst stage.
  8. Embryo transfer: Finally one embryo (sometimes two) is placed gently inside the uterus, either fresh on day 5 or later in a frozen cycle.

What Does ICSI Cost in Gurgaon?

In Gurgaon, ICSI cycle usually costs anywhere from ₹1.5 lakh to ₹2.6 lakh for one cycle. The range is wide because the cost is not a fixed thing. It changes from one clinic to another. It depends on the medication protocol used for you.

And if you need something extra, like surgical sperm retrieval or PGT, the cost goes up further. So the only way to know your real figure is to sit and discuss your case. It depends mostly on your protocol and on how your body responds to the medicines.

Why Choose TheFertilife for ICSI Treatment in Gurgaon?

  • The Fertilife offers ICSI (Intracytoplasmic Sperm Injection) as part of its IVF program for cases where fertilization is unlikely to happen with standard IVF alone — including male-factor infertility, low sperm count or motility, and previous fertilization failure
  • Each ICSI cycle starts with a semen analysis and sperm selection process to identify the healthiest sperm for injection, rather than defaulting to ICSI for every IVF cycle regardless of need
  • Embryos created through ICSI are monitored through the same fertilization and development checks as conventional IVF, so quality control doesn't change just because the fertilization method does
  • Protocol and medication planning is adjusted to the individual — ovarian stimulation approach, egg retrieval timing, and embryo transfer decisions are matched to age, ovarian reserve, and the specific reason ICSI was recommended

Ready to Find Out If ICSI Is Right for You?

You really don't have to know already whether you need ICSI or simple IVF. That is exactly what the consultation is for. Just bring your semen analysis and your fertility history, and you will leave with a clear, honest recommendation built around your case – not a default upsell.

Frequently Asked Questions

Not automatically, no. ICSI raises fertilisation rates only when there is a male-factor problem or a previous fertilisation failure. For couples without those issues, large studies show no real benefit over normal IVF.

The egg retrieval is done under short general anaesthesia , so you feel nothing at the time. A little mild cramping afterwards is normal. As for the ICSI injection itself, that happens in the lab on your eggs, so nothing at all is done to you at that step.

Very few. Far fewer than normal IVF, or even natural conception. ICSI needs just one good sperm per egg. That is exactly why it is so often the only option for men with very low counts or for those who need surgical sperm retrieval.

No, and any clinic that promises you a guarantee is not being honest. ICSI improves the chance of fertilisation when there is a clear reason for it. After that, embryo growth, the transfer and the implantation each carry their own odds, and Dr. Lekhi will walk you through yours.

It depends on your protocol and on whether you also need extras like surgical sperm retrieval or genetic testing. Add on ICSI can cost between Rs 10000 -20000.Call or WhatsApp +91 9560026697 for your own estimate.

Mostly, children born through ICSI are healthy. Some studies do note a small rise in certain risks compared to natural conception, similar to what is seen with IVF in general, and this research is still going on. Dr. Lekhi can explain the current evidence to you in your consultation.
